Policies were opened *****ulently in my name at Old Mutual. The identity document used contains my ID number and names, but the photograph is not me. This is clear evidence of identity *****. I did not apply for, consent to, or sign any documentation for these policies. Monthly deductions are still being made directly from my PERSAL salary, even after I reported the matter and requested policy cancellation.
