Please, please, please give me a correct IRP5

I've been trying for 12 months to get a IRP5 out of Old Mutual for a Retirement Annuity policy I have surrendered. Last year Old Mutual submitted the IRP5 to SARS with the incorrect information by omitting the tax they automatically deducted from the policy. Now SARS claims I owe them money, but Old Mutual did deduct the money, but did not reflect it as per the tax directive. Every time I call or email the customer service department the people have no idea how to help me. All I want is a IRP5 reflecting the money they already paid to SARS.
