Old Mutual tax free savings

I opened a tax free investment account for my daughter and was paying monthly to save up for her this year. I then withdrew on the 22 December 2017, for me to get an email on the 5th of January 2018 that my claim was rejected because I didn’t provide my 5 year old’s bank account. Yet this was not told to me when I was taking this option not when I was withdrawing. I am really disappointed.
