Old Mutual Retirement Fund - A finalized tax directive was received but no lump sum was declared

I am an ex Nedbank Employee. I resigned from Nedbank on 31st July 2013. This is been on ongoing issues for the past few years. When i try to file my tax return for year 2014 i get a message from SARS that i tax directive was issues by Old Mutual but no lumpsum was declared. I have been requesting a copy of the tax certificate to no avail. The directive number is ***9. I have been incurring admin penalties from SARS for all these years due to the inability to get the issue resolved.
