Old Mutual - Please send me my documents.

I was an employee for old mutual in 2017. I resigned. I now need a UI19 form to be sent to me, so I can be able to claim UIF, now that I’m unemployed. It’s been 9 months since I sent my first email to Old Mutual to provide me with this document. I have still not received it. Please assist me with this query.
