Old mutual Phalaborwa

My Dad recently past on two weeks ago,and he had a retirement plan with old mututal in Phalaborwa,last week friday we went to gyani old mutual with my three sisters and my mother and his second wife,where my mom was told that shes the first beneficiary wife,and we were given forms to fill up and send to capetown,on monday my mom went back to oldmutual were she was told that shes not the beneficiary wife anymore and her for kids are not included on the will,his second wife is the beneficiary wife now,she requested to be shown the will and they told her they dont have access to it,how does all this happen if its not ***** from the once in old mutual Phalaborwa,somthing can't just change,making other people suffer for there own benefits
