Old Mutual not supplying forms to release annuity money

I have been struggling for almost 2 months just to get a Transfer of investment form after making my annuity paid up. It has been up and down, call after call, escalation and so forth with no success. I have learned that this is Old Mutual's normal way of business from others who have had the same experience. It is really terrible service. One would think a big company like them would/could do better.
