Old mutual *****

I'm having a problem with old mutual someone keep on opening policies with my personal information at first they called me to confirm that I opened the policy, I told them I did not open any policy with old mutual someone it's using my information without my permission I asked them not to open any policy for me.

After a month old mutual policy deducted on my pay slip I called them they said I must fill in ***** form and email it to them I did so.

Each and every month old mutual its opening new policies for me I cancel them but still they keep on opening new once. I'm tired of this every month I should cancel a policy. I did wrote an email to them that if there's anyone who is opening policies with my ID and persal they should contact me and confirm if it's me, but still they not contacting they keep on opening new policies some times they open two or one policies I'm tired of this please help.
