No final statement for my investment

I had a maxi investment with Old Mutual in which I disinvested because I had reached the maturity date. I got the money but I never got a statement because I wanted to see how my investment performed. I have asked the service consultant 3 times but I still haven’t received it…it’s almost a month in now. I’ve also tried to call your service line but the phone just rings and rings
