Insuring the incorrect person .

My dad has dealt with Old Mutual for over 20 years , he recently lost his job as the company was liquidated. He then contacted our broker and his assistant assisted my dad to fill out forms to drop my mom off the policy as he couldn't afford to pay for her but only for himself . He fell ill in December 2022 , we lodged a claim for disability insurance as that's what's included in his insurance. However ,they lodged the claim and now they saying that they dropped him out and insured my mom, as well as before this ,the lady had insured my Aunt instead of my dad . I have the documents on hand of the mess up she made. I have requested the documents of which my dad signed however still to date they have not given us the documents. I need Old Mutual to come to party and assist us as we've dealt with them for years and this is very unprofessional.
