- Certified copy of your identity document - Bank stamped bank statement (not old than 3 months) - Your tax reference number

I know the winding up of the estate has been concluded and all funds and documents have been turned over. I just wanted to find out when the monthly income from the below mentioned Old Mutual policy will be received? I haven't started receiving it and have sent information required multiple times.

Since so much time has elapsed, will there be any backdated income received?

In order for the 2nd life assured to received the monthly income we require the following.

1. Copy of the 2nd life assured identity document of Mrs M Mkhize 2. Income tax number to be issued by SARS 3. Proof of banking details in the name of Mrs M Mkhize in the form of bank statement or letter from the bank signed and stamped by bank official
