I recently went back to Old Mutual regarding a case that I open last year October,about a policy they opened for me without my consent/permission only to get the same response that it's still with their forensics department

I recently went back to Old Mutual regarding a case that I open last year October,about a policy they opened for me without my consent/permission only to get the same response that it's still with their forensics department. My question is how did they open a policy for me without my consent and how did they get my information because I never gave them any?. And every year whenever we go for deployment we come back to find that on our paysheets there's a policy that's from them.
