My name is Mahlatse I am having a problem with Old Mutual and I need your assistance

To cut the story short, My late mother was a policy holder. She nominated my dad as her first beneficiary, after some period of time she went back to Old Mutual and removed my dad then nominated me and my 2 brothers as beneficiaries. Then she passed away last year. We went to Old Mutual to claim as we were beneficiaries. They told us their consultant did not submitt my mom's second request of which we have evidence and their system can also find us. We tried 2 different Old Mutuals since last year June until now
