Funeral policy opened without my knowledge.

Iam so annoyed with old mutual funerals .i jave a comprehensive cover ,that coves mi immediate family and my mom and siblings since 2010.today i recieved an email stating that i have opened a policy for my mom ,with different amount ,i have never spoken to anyone from old mutual with regards to that forged policy. My home address have been chanced, my mom date of birth also changed i need to find out who using our information to open policies without our consent.worse part of it the premium is a stop order.they even accessed my persal without my consent .old mutual has poor service ,i want their policy cancelled ...whoever opened a policy frodulently for commission must account .My no is *** .i am going to the branch 1st thing monday to cancel whole funeral cover .il rather go somewhere ...
