********** nomination form (After deceased past away)-Ref no: R001499D

The deceased took up a pension cover with old mutual which is payable in a event of death, The deceased left a nomination form of beneficiaries or dependants and it was allocated with certain percentages. The nomination form has the percentages and it clearly states which apportionment percentages needs to be apportioned in accordance to the deceased wishes. However an old mutual administrator has decided to ignore the nomination form percentages and take disregard of the nomination form percentages. The old mutual administrator has shown bias in apportionment of the benefits to the dependants in such a way that it shows poor service in regards to Old mutual. The old mutual administrator claims that a trustees have decided to edit the percentage shares, however this is unacceptable and it represents an unfair practice as the deceased had left the nomination form with certain percentages. The reference number that Old mutual gave for this case is R001499D. I have since phoned old mutual and asked for a review of this case. However the old mutual administrator allocated to this matter is still showing bias and is not transparent in communicating these issues or even explaining why the trustees decided to go against the deceased wishes. I would like to get help in making sure that the deceased wishes are carried out in the way that he had left them to be and not in accordance to the biased Old mutual administrator. The old mutual administrator claims that this policy was carried out in accordance to section 37C but they have contravened this act. By apportioning a lesser percentage to a grandchild that was nominated by the deceased and taking disregard to the nomination form.
