********** deduction in my payslip

Two weeks ago I saw on my payslip that there is a deduction for old mutual group schemes of R352 and I never took a policy with old mutual group schemes can Old mutual refund my money and cancel this policy because instead of refunding me all the premiums from April to June they refunded me for one month premium only and when I check on the system this policy has not been cancelled. On the 4th June 2021 I went to East London Vincent Branch and opened a forensic investigation case up to today no one has contacted me I need to know who has opened a policy for me without my consent??
