Failure to deposit cash back

Every month Old Mutual debits my account, I have been paying them consistently for years now. I was told I'm due for a cash back...suddenly they don't know which account to use to deposit the money and they are always "escalating" such a simple matter with no resolution. I am genuinely not impressed
