Double deducting monthly premiums

I am writing this in behalf of my mother. My mother has a policy with Old Mutual for more than 10 years with no issues but for the past 5 months they have been double deducting her monthly premiums. She went to Old Mutual offices several times trying to resolve this issue with no success and mind you she is a pensioner. Her initial monthly premium is R122.00 and they are deducting R248.00 . I feel like Old Mutual is not taking further steps to resolve this issue, they are robbing an innocent pensioner her money and she is spending unnecessary taxi fare every month because she has to take a taxi to go to their offices to try and resolve this issue.
