Dispute of unknown accounts

I have manage to check my credit report, only to find out that one employee of old mutual has manage to do a credit check under my name twice on the 22 October 2020. I which am not happy about it because is seens this person wanted to *****. And the boring part I did not go any of the old mutual branches around South Africa or even call the head office requesting for the credit at old mutual. Or in this case they have to let know which branch or send me a voice record or any other proof to show that It was me how did a credit check.
