Disinvestment/ withdrawal from olld mutual

Good day, because I broke my leg and needed money for medical expenses, I requested to make a withdrawal from the short term savings pocket of my old mutual group schemes. I completed the transaction request form and scanned it and emailed it together with my ID, proof of residence and bank statement to ***. Thhey requested that I COMPLETE a Disinvestment FORM which I completed and emailed on 13 April 2022. i had requested a withdrawal of R650. I have not received any money. i was discharged from hospital on TUESDAY 3 May and I am only now well enough to send this email. PLease can you help me to get this money. many thanks
