Dishonest , and unprofessional

I'm highly disappointed and frustrated at old mutual, I opened an investment in 2016 every month they deduct directly from my work , im a nurse and it shows on my pay slips, I went to try get some funds they say they can't see the value of the investment , but made me sign a surrender form that they can't process due to unknown value , I requested statements via telephone to be emailed they said they only send statements once a year which I have never received, and didn't not send any statements , 1st they ticked me into 10 year investment, when initially the consultant said it was a 5 year investment , il will be lodging a formal complaint
