Disappointing service

Old Mutual refuses to reinstate a retirement policy that just ceased at its 5th year anniversary. It was due to get a bonus and they say it's lapsed with 3 months missed premiums.

I asked them to debit all premiums but im being sent from pillar to post. I've filled reinstatement forms and debut order forms etc.. to no avail.
