Disability claim not paid

I was in a car accident in September 2021 and my right leg above knee was amputated. I have disability insurance for lost of leg above knee from old mutual. Last week they only paid R5165 as a refund of premiums. To date I’m still waiting for my disability claim to be paid out. I had a waiting period of 6 months which past already. I’ve send all supporting documents for the claim as requested. Why will they pay out my premiums and not my disability claim as I need the money to pay for my prosthetic leg as it’s very important for me to have my prosthetic leg. As you can see below I have disability benefits
