Debited a deceased account and opened a new policy using deceased information

Both me and my mom had funeral policy with old mutual,so Eunice passed away in 2021 I claimed and since they had access on her account,someone at old opened a funeral policy using my mom's and the debited an a deceased client account when I went to query this no valid response I got because this issue was still sensetive to me I decided to leave it until am ok.Old mutual is becoming something else.I don't want to ever work with them.
