Debit order change

My mum and I have visited the branch at midlands liberty mall on the 20th of April. We want to change debit order banking details from my brother who has relocated to my account. It is now June and my brother is still being debited, we have visited the mall three times since , filled in debit mandates and forms even emailed old mutual with no response. We have been loyal customers and we demand good service.
