Death claim forms

We have been sending death claim forms back and forth for over 2 years. we receieve no feedback if there's missing documents until we have to inquire. we (me and my siblings) have sent in new forms (which we have been filling over and over again) and received the folowing reference CCC ********** 4.

Please assist where you can

Is policy number ********** 42 supposed to pay out as well or is it just a Tax Residency Declaration form?
