Good day.

I am writing to formally lodge a complaint regarding an unauthorized Old Mutual policy that has been opened in my name. I have discovered that this policy was joined without my consent, and it appears that my personal information and signature were *****ulent.

Despite my numerous attempts to contact Old Mutual to resolve this matter, I have not received the necessary assistance. This unauthorized policy continues to debit funds from my account every month for a service I did not sign up for and have no knowledge of.

I am requesting an immediate investigation into this *****ulent activity, the cancellation of the policy, and a full refund of all premiums debited to date.
