Nu metro refusing with my forms to claim for uif

I was working for Nu metro at Ballito Junction, i got dismissed it's been over 3 months now they are refusing to give me my forms to claim for uif. I haven't been receiving any income for the past 3 months and i have kids to take of, i have tried several times to communicate to the manager but he's not helping me. I believe i was supposed to get those papers as soon as i was dismissed but i still haven't got them.
