Questions re Netcareplus gapcover

I am disappointed. We had surgery in the family. Netcareplus insists that I complete the online form, but it does not work. I finished the forms manually, scanned all the required documentation, and mailed them to the address supp****. I never had any feedback except for sending me another standard form. What worries me is the warning (in RED) that this form was created outside Netcare and I must be cautious
