After many phone calls and communications between me and Customer Care of NBCRFLI health and Affinity Health nothing had happened over 5 months of struggling. <br> <br> We did send though my spouse's ID- and Marriage Certificate so that my wife can be registered on the system and receive a membership card with her name on the Card. <br> <br> Today I received a memership Card from you with only my Name and ID- number on the Card. This fund is adminstrated by affinity health.
