Never dealt with the company but they incorrectly placed me under debt review

I have never dealt with the company but they have gone ahead and submitted to NCR that I app**** for debt review with them, negatively affecting my credit score. How does a debt counselling company, without your consent or any documentation, put your name under debt review?
