Bad communication

I have been sending countless emails and calling only to hold for 20 minutes and more without anyone replying back. I am requesting to be taken off the program and haven't received feedback nor acknowledgement. i only need a form to fill so they can take me off the program as my credit shows that am listed under them and i have stated the reason why i want to be taken off.

my car is written off so i wont be paying for it and plus the insurance so i can now afford and want to apply for consolidation with my employer.
