Registered under debt review with 2 debt counselors

Good day, I app**** for debt review with NDA in 2017 but I later withdrew from them and on that same year I was app**** with Octogen and I received a clearance certificate this year. I only found when I app**** for credit at Capitec and ABSA that my name is still registered under debt review with National Debt Advisors. I contacted NDA but they said that they have no records of my name being registered under debt review with them. Kindly assist me what to do in this regard
