National Debt Counsellors

Nov 2020, I made a mistake of registering for Debt review, in less than 2 weeks I went back to the debt councilor, and I asked to be withdrawn. They confirmed voluntary withdrawal they gave me form 17w They have been refusing to give me form 17.4 confirming that I'm no longer using their services. I ended up get cold shoulder and not been responded to , I was dealing with Chantal Mans. This has been messing my life as I'm not able to do anything due to this.
