I want my policy canceled

Can someone please cancel my policy? I have called numerous times with no assistance. Two of your agents promised to send me a form to fill and nothing was sent to me. I called again and someone kept dropping the call on my ear.

I want this policy canceled.

I have not received any feedback from MVIA, no one from the cancellations department has called me. I do not want any further debit orders to go from my account, I want my policy cancelled.
