Unauthorized insurance

i opened an account with mr price in 2017, withing a month of it being active i received a call from one of their consultant offering me FAMILY FUNERAL OPTION 1 R 43.00, i declined, the next week another call came through, and i again declined, got my statement with the funeral insurance included, i called in, i was told to go to the nearest mr price and cancelled, i went to randburg and they told me they didn't have the forms, i then went to cresta mall, same thing, my worry is that i've been paying for something i never authorized, and being sent from pillar to post for something i didn't is not fair at all, yours calls are recorded, i told that guy that i don't want it and i want it cancelled and be refunded for the months it was added on my statement.
