Fed up by filling in forms and nothing happens

I am not happy at all with Vaal Mall's Mr Price's staff. I have been there twice filling out forms to close my account and both the times they tell me they will fax the form to the head office. Still they send me a statement every month to pay the customer protection plan on my account. So I don't know what is the hangup with my account not getting closed. I don't know how many times must I go by the shop to fill in forms to close my account. I am really not happy with the service of the Mr Prices personnel.
