Bad attitude and service

I had an account with Mr price. On 14th December we paid the account off and asked them to close it. Filled out all the forms and the lady behind the till said it was sent to head office. A month later I received a statement saying I owed money for insurance that was placed on the account, I phoned head office and was told that they had not received any paperwork. Trying to follow up with the store after being placed on hold for 10 mins plenty times I was told it was sent through to head office. Now it's the same issue again. Really not impressed with the service and way they handle their accounts.
