Personal and confidential information shared without consent

I received a call from Cubix Solutions on behalf of MiWay. The agent who called had my personal information which he used to question me about my /motor vehicle insurance. I have never consented to my personal information being disclosed to any party. Apparently I took a survey where I consented to this, but the agent could not provide any evidence thereof. I request that MiWay remove all my personal data from your database, as well as from any third party who acts on behalf of MiWay.
