sick and tired

good day. my husband resigned 27 Feb 2015 from a company he worked for. We then filled in forms for MIBCO to pay out his Provident Fund. as we called them today they said that they have received claim forms for last year 2 July 2014. we never sended claim forms for last year. I don't know where they got the forms from. as I spoke to the supervisor she got all bad attitude and she started to be arrogant. I asked for a email where we can confirm the claim on the date for the 2nd July 2014 and she said she wont send me the claim form and she hang up in my ear. this is not on. I am and will take this further.
