Onions in order even after no onions requested on MrD

I ordered through MrD for Nachos and requested no onions as I'm allergic, staff obviously did not check the comments on the system of no onions, after trying to pick them out I gave up as they cut red onions into tiny pieces in nachos and salsa and I had to throw out the food as I was not carrying an epipen and wanted to avoid suffocating. I have been trying to get Mr D to investigate as I did not take pics of food before throwing it away and the food was cold like it was left over in the fridge that was reheated, while digging out onions I felt ice cold tomoes and cottage cheese, I would like this to be be investigated by restaurant owner or manager as well as an apology
