No one wants to assist in cancelling the policy.
I have been calling and going to the branch asking for metropolitan to help me cancel one of my hospital policy with no success. I am kindly asking you guys to cancel the policy.

(c) reason for the cancellation
(d) date the letter was written on
(e) signature of the policy owner

2. certified copy of your ID document with 3x specimen signatures on the document
