Still waiting for forms to be emailed

I called your call centre on 6 August and spoke to Betty Gqabaza who wasn't very helpful. Had to explain myself over and over again to get her to understand that I needed information on how to add additional members to my existing medical aid. Eventually she said she'd email me a form to complete and return. I'm still waiting for the form. I completed the survey and stated that I wasn't happy with the service received. I was called back yesterday (12 August 2015) to follow up on my feedback and was again promised that the form would be emailed with a quote for how much extra it would cost and I still have not received the form. This is quite urgent and I am getting nowhere. Please get someone to send me the requested form and information ASAP. The original query number is 060815QF1S0D
