COMPLICATION FOR MEMBER TO SORT OUT
MEDICATION IS ON THE FORMULARY AND IS A BASIC LINE OF TREATMENT YET THEY REJECT IS WITH A CO PAYMENT STATING OUT OF FOUMULARY MEDICATION...
YOU PAY SUCH HIGH FEES FOR ****

Upon the investigation we can advise as follow , Mediplus option chronic medication is on restrictive formulary list.Medication in querying ( Dyna Sertraline ) is out of formulary hence it was processed with co-payment.
