Bad experience from medical aid

I have been using this medical aid for over 14years, I have received absolute service that gives a member like me a peace of mind. However I experienced trauma in 2021 whereby my late wife passed on though being given the decent care at Life Carstenhof. My wife was admitted for Covid-19 related complications and stayed in hospital until she passes in the ICU, I realized later that medical aids had not settled payments and this had caused me to fight with the service providers and it's frustrating especially when coming to the issue of the pandemic. Upon following up with relevant service providers, I came across another long outstanding account that medical aid never paid when my daughter was admitted in the Life Carstenhof in 2020 which had been handed over whereby the medical aid never settled, I am disappointed of this behavior as it has caused me alot after my loss and it is so stressing, worse is that I notice that medical aid had been deducting full premium that of which includes my late wife still as dependant and when I quiried I was informed to send documents as they don't know about the patient being past on, after I made what was required to remove my late as dependant, I was informed further that the premium would not be backdated since August 30 2021, I found it improper but I never follow it up to argue the matter. Now my question is that 1. Why is it that medical aid did not settle my daughter's account bill of R2000.00 2. Why is it that my late wife account not settled to service providers when I believe that PMB would cover in hospital after patient exceeded 7days in hospital? 3. Why is it that medical aid deducted premiums from August 2021 until the April/May 2022 I enquired refused to back date the premiums or address it otherwise? 4. How is that medical aid did not know that my wife passed on, which raises questions to me because, hospitals does notify or report to the medical aid about the progress of the patient? I lost hope as all debts are now facing me and which are caused by medical aid and my credit reputation has been jeopardized.
