***** Claim for service that was not rendered

i am a member of medipos medical aid , i have been for the past 5 years out of nowhere all of sudden i receive an email with the claim that i know nothing of, a doctor by the name of GLADSTONE BALOYI made a claim for the service he did not render ,and i believe authorization should be requested for such ,so i reported it with whistleblower which it was resolved but the same doctor made another claim same day , my question is was he not supposed to have been blocked or blacklisted from making claims as i have reported that he is a *****ster , and also who else in the company suddenly licked my information to him and who keeps processing it without checking properly
