Lost tooth during the procedure

I was admitted at Highveld Mediclinic on the 13th October 2017 to remove abscess from my right hand. On the day of the procedure I was given a consent form to complete and declare anything relating to my health, and I waited for my turn to go the theatre as planned. I was then put to sleep for the procedure. I then woke up in the other room out of theatre around 20:15, I was then taken back to the ward, I was still feeling sleepy. When I got back to the ward the nurses gave me something for pains, I then fell to sleep. The following morning when I woke up at about 05:30 I felt something strange in my mouth, I took my teeth brush to brush and wash my mouth, immediately when I started brushing I saw some small drips of blood and I noticed when I look in the mirror that one of front lower jaw tooth is missing. I carried on bathing and waited for Dr Hoosen to come for his rounds so that I can ask him what happened or if he knew anything. Dr Hoosen said he did not see anything but his assumption was that when the theatre staff inserted the steel air pipe in my mouth maybe something went wrong but he said he will find out. The sister in charge at orthopaedic ward came to ask me about what happened, I explained to her and she promised to get some answers from the theatre staff. On Thursday the 19th Thursday Dr Jaca came for the first time to ask me what happened, I explained again to her the same scenario. Her answer was that she doesn't know what happened, I was left wondering and amazed by the way things are done at this hospital, you go for something else and come out with the missing tooth. I sent an email to the CEO of the hospital to look at the matter he then referred it to one of the regional managers in Midrand (Joey Le Roux) to do her investigation. The feedback I got from Joey was that the hospital cannot take the responsibilty for something they don't have proof of what happened. They are the one who put me to sleep after I have signed the consent form to remove the abscess from my hand and not the tooth from my mouth, and then they tell me "they don't know what happened"
