Double quarter pounder incomplete.

I just went to a McDonald's to order something to eat. I ordered a double quarter pounder medium meal and the were no onions and gherkins in the burger. The only special request I made was to only request that there be no ice in my drink. I was expecting everything else to be standard. I only realised the problem after I left the area where I bought the meal. I am disappointed with the experience as this mistake has never happened before.
