Cost saving at customer 's expense and risk

1. I was made to buy ketchup for 50c
2. The drink we ordered to eat in came with no lids.
3. The serviettes were small because they were cut in half
Only for one staff member whom I will not mention for their safety, said that they complained about all these and they were told that the store is saving costs by doing that.
I am shocked that I had to buy ketchup and that purr hygiene is risked by not closing our drinks because the store wants to save costs..
When I complained to the staff member, I was begged not to call the manager because they will be fired.
I demand a full explanation on all this or I am writing to Macdonalds America
