cellphone insurance claim

I insured my child s mobicel tab but now is lost i went to markham highveldmall and they always send me back without taking my details. Is morethan a month now and the last time i went it was yesterday 23 August where they told me to comeback again today monday. they dont want to process my claim because i dont have the original slip that was given to me when i bought the cellphone, but they keep on deducting insurance monthly from my account

It is 19 months now, still unable to cancel the insurance from markhams store, i caleed they reffred me to the shop, i went to Markhams Kwa Guqa they said they wont be able to help me with cancelation of the insurance because the phone is lost, i dont have the original slip nd i didnt buy it from their shop. I then went to Markhams Highveld mall still the same problem, but the R53 isurance keeps on getting deducted for nothing. the last time i went to the store it was on the 13th April still they asked me to travel to Benoni Lakeside mall because they dont have my info i didnt buy the phone from them. how can i travel all the way from Benoni just to cancel insurance? i had a loss in August 2015 they didnt want to help me because i bought at Benoni , i tried to cancel the insurance, still they cant help me because i bought at Benoni, it means i will pay this insurance for the rest of my life until i die.

I need written feedback regarding this and The next option will be to contact the short term insurance ombudsman regarding this, maybe they will save me from this ordeal.
